Description and Responsibilities

The North County Emergency Dept. Diversion program provides short-term (30-60 day) community support to individuals in the process/recently discharged from either Heywood Hospital or Leominster Hospital. The goal of the program is to help decrease boarding times and/or recurring visits to the Emergency Department. This program offers support to individuals across the lifespan – ranging from children and adolescents to elderly. The E.D. Diversion program provides clinical, case management, and peer support services to individuals experiencing an increase in mental health symptoms. Team members assist individuals in connecting with community resources, service providers, and provides crisis support/intervention. This program is funded by a grant through the Department of Mental Health.

 

The Outreach Counselor provides service coordination for individuals served, including employment and education, emergency service providers, medical and behavioral health referrals, outpatient clinics and others. They also assist individuals in applying for, and accessing eligible benefits entitlements. 

 

Other Key Responsibilities: 

  • Deliver interventions identified on the Treatment Plan with supervision from the Director of Respite Services.
  • Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
  • Assist in obtaining reliable transportation, and/or provide transportation as needed.
  • Intervene during a crisis, following established program and Agency procedures.

Qualifications

  • High School Diploma, GED, or equivalent.
  • Experience working with children and/or adults with chronic and persistent mental illness, preferred.
  • Valid Driver’s License, reliable and insured vehicle, and acceptable driving record, required. 
  • Reliability, willingness to learn, and being open to new opportunities.
