Adult Community Clinical Services (ACCS) is a Department of Mental Health (DMH) funded service that is designed to deliver evidence-based interventions within the context of a clinically focused model. Individuals served reside in the community, both in their own homes and in group living environments (GLEs). ACCS Teams are multidisciplinary, and include clinicians, substance abuse counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff.
Team Leaders provides leadership to the Integrated Team as well as direct supervision and support to Outreach Counselors, the Housing Coordinator, and the Administrative Assistant.
The Team Leader will:
Facilitate and monitor the implementation of activities that create opportunities for the development of valued roles and personal relationships in the community.
Assist people to access, and/or provide, all needed transportation.
Participate in On-call Beeper rotation.
