The North County E.D. Diversion program provides short-term (30-60 day) community support to individuals in the process of being discharged, and those who have recently been discharged from either Heywood Hospital or Leominster Hospital. The goal of the program is to help decrease boarding times and recurring visits to the Emergency Department. This program offers support to individuals across the lifespan – ranging from children and adolescents to elderly. Services provided include clinical support, case management, and peer/family support services to individuals experiencing an increase in mental health symptoms. Team members assist individuals in connecting with community resources, service providers, and provide crisis support/intervention.
The Family Partner will provide hope, information, advocacy, and support to families of individuals with mental health issues that are receiving services from the E.D. Diversion Program. They also coach families in developing advocacy and collaboration skills as well as empower families to understand and exercise their rights.
Key Responsibilities:
High School diploma, GED or equivalent, required.
Valid Driver’s License and acceptable driving record, required.
Experience being a parent, caregiver, guardian, grandparent or foster care parent whom directly supports a person with mental health challenges, required.
Current or past experience accessing mental health or related services for that person, required.
