Description and Responsibilities

Open Sky Community Services, in collaboration with the Department of Corrections, is excited to announce a first of its kind program in the state of Massachusetts, The After Incarceration Support Program. This program will support the re-entry of returning citizens who have been recently released from the Department of Corrections and other individuals who have a history of Justice System Involvement. The Aftercare Incarceration Support Program is community-driven and fosters active community involvement, and engagement. The program offers a safe, judgement free, resource rich environment that breaks down barriers to reintegration while providing access to essential, customized resources, and supports a mission to invest in each individual’s success. The scope of services includes outreach and engagement, food and nutrition assistance, educational and job placement services, housing assistance as well as healthcare and addiction treatment.

 

The Care Coordinator Navigator uses their experience with local housing resources and benefits resource to provide care coordination and referrals to individuals served at the program. They coordinate access to services and fulfillment of integrated health needs including behavioral health and medical appointments.

 

Other Key Responsibilities:

  • Develop an understanding of the behavioral health insurance system network and coordinate enrollment with individuals to their health plan of eligibility and choice.
  • Work collaboratively with justice entities, health care providers, and community providers to integrate care and address social determinants of health.
  • Secure outpatient appointments for individuals and facilitate home and community-based services as necessary.
  • Facilitate communication between the member or designated representative and member’s healthcare providers

Qualifications

  • High School Diploma, GED or equivalent, required.
  • Valid Drivers License with a reliable and insured vehicle and acceptable driving history, required.
  • Minimum of one year of experience working with adults experiencing homelessness with serious and persistent mental illness and substance use disorders, required.
