Description and Responsibilities

Open Sky’s Rapid Response Hub provides urgent and timely access to emotional, mental health and behavioral health services, all starting in one place.  Individuals are coupled with a member of our team, who use the “whole person” approach to address a variety of needs including behavioral health, substance use, navigation of the medical system, housing/food insecurities and benefit enrollment. The Rapid Response HUB focuses on providing care to individuals in traditionally underserved Latinx, LGBTQIA+ communities, Veterans, Justice-Involved and people experiencing housing insecurities. Anyone seeking services can make an appointment and no referral is required.

 

Targeted Case Navigators are caring, organized, creative, and persistent.  They engage with the individual in need and support them with gaining access to treatment services and community resources.  They develop and assist in implementing support plans in collaboration with medical and behavioral health providers and other applicable community resources. They play a critical role in facilitating a fast turnaround for the most dire of situations.

 

This is a regular full-time, benefits-eligible position.

  • Engage with individuals as they enter the treatment system and support them in gaining access to treatment services and community resources.
  • Complete assessments. Develop and implement support plans. Record progress. Complete all documentation within a timely manner.
  • Act as a liaison between members, community agencies, and families to coordinate treatment, including: employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
  • Exercise good judgment. Recognize and respond appropriately to risk-related situations.  Communicate/ask for support from the team and program leadership when needed.
  • Provide temporary transportation assistance as needed.

Qualifications

  • Bachelor’s degree in a related field preferred.  Alternative education/experience in lieu of formal education will be considered.
  • Experience working with homeless, justice involved, adults with serious and persistent mental illness, substance use disorders, and/or underserved populations preferred.
  • Ability to stay organized and prioritize while managing a dynamic workload.
  • Creativity and resourcefulness.  Knowledge of local resources and experience navigating treatment systems preferred.
  • Reliability, drive to learn, and compassion for those who are struggling.
  • Valid Driver’s License.
  • Bilingual-Spanish applicants, highly desired.
