The Director of Complex Care Integration is responsible for ensuring coordinated, trauma-informed, whole-person care for individuals with complex needs across the Community Services Division and the broader organization. This role focuses on reducing service fragmentation, strengthening cross-program collaboration, and improving care transitions for individuals involved in multiple systems.
This position maintains a small, specialized caseload of high-risk individuals enrolled in the Community Support Program for Homeless Individuals (CSP-HI). In addition to direct service delivery, the Director serves in a systems-level role to support integration, program alignment, and effective service utilization.
