Description and Responsibilities

The Integrated Dual Disorder Treatment program is an evidence-based, wrap-around service model for individuals with a dual-diagnosis of a mental health and substance use disorder. The program works with individuals by providing individualized, community-based support from a highly integrated, multi-disciplinary team including a clinician, navigators, recovery coaching, supervisory staff, nursing, and psychiatry. Individuals are provided support through person-centered treatment plans in the settings where they are most comfortable, including the home or other community locations, with the goal of empowering individuals to explore the various pathways to recovery for both mental health and substance use. 

 

The Case Manager will be responsible for supporting individuals with accessing and engaging in healthcare services and community resources. They are caring, organized, creative, and persistent people who serve as an advocate for individuals in navigating the insurance and healthcare system and in overcoming barriers to accessing and receiving person-centered healthcare.

 

Other Key Responsibilities:

  • Providing case collaboration with an interdisciplinary team regarding individual’s healthcare and medications.
  • Exercising good judgement by recognizing and responding appropriately to risk-related situations with communication to team and program leadership.
  • Provide temporary transportation assistance as needed.
  • Completing timely and appropriate documentation according to agency requirements.

Qualifications

  • High School Diploma or equivalent, required. 
  • Valid drivers license,acceptable driving record and reliable transportation, required.
  • Experience with providing person-centered, trauma-informed services to adults with mental health and substance use disorders.
  • Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems, strongly preferred.
  • Effective written and oral communication skills, and the ability to function independently with minimal supervision.
