The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection.
The Enrollment and Care Coordination Specialist will screen indivdiuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboartion with the team lead, as needed.
Other Key Responsibilities:
High School Diploma, GED or equivalent, required.
Minimum of two years’ experience with housing and behavioral health population, required.
Valid driver’s license and acceptable driving history, required.
Must be able to collaborate with a multi-disciplinary team, possess excellent written and verbal communication skills with attention to detail, required.
Experience with electronic health records, billing and/or referral management are strongly preferred.
