The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection.
The Housing Team Lead operates within the philosophy of the Housing First Model to assist individuals in meeting their housing needs and preferences. They assist in facilitating housing searches, acquisition of permanent housing, and work as an advocate with vocational programs, employers, therapists, doctors, and social security to support persons served as needed. The Housing Team Lead is also responsible for hiring, supervising, and mentoring the Housing Specialists within their team.
