Description and Responsibilities

Open Sky’s Rapid Response Hub provides urgent and timely access to emotional, mental health and behavioral health services, all starting in one place.  Individuals are coupled with a member of our team, who use the whole person approach to address a variety of needs including behavioral health, substance use, navigation of the medical system, housing/food insecurities and benefit enrollment. The Rapid Response Team focuses on providing care to individuals in traditionally underserved communities, Veterans, Justice-Involved and people experiencing housing insecurities. Anyone seeking services can make an appointment and no referral is required!

 

The Case Management and Intake Supervisor will provide immediate intervention and support to individuals seeking services in the Rapid Response Hub program.  They will meet with individuals either in person or via the telephone to gather information about their needs and current resources and provide information about programs and services both within Open Sky and the greater community. They will assist the individual in gaining access to needed services and offer ongoing support as the individual engages in the services they have chosen. The Case Management and Intake Supervisor will also provide supervision to the Targeted Case Managers.

 

Other Key Responsibilities:

 

  • Review referral documentation submitted through phone, fax, email, triage tracker, or other means to connect with referral source and/or referred individual to determine eligibility for services.
  • Assist with data entry for the SAMSHA grant, proper documentation to meet standards, complete NOMS baseline assessment, reassessments, and discharge summaries.
  • Track Individual’s progress through the system and problem solve for barriers as they arise.  Maintain outcome and data reports.
  • Complete all Assessments and Support Plans according to prescribed timelines and specifications.
  • Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.

Qualifications

  • High School Diploma, GED or equivalent, required.
  • Minimum of 2 years’ experience working with homeless, justice involved, adults with serious and persistent mental illness and underserved populations, required. 
  • Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems strongly preferred
  • Valid Drivers License and acceptable driving history, required.
