  • Responsibilities

    1. Provide orientation, training, direct supervision and evaluation to staff as assigned.
    2. Lead shifts and provide direction and guidance to other program staff.
    3. Assist the Program Coordinator in the general running of the program. 
    4. Assist the Program Coordinator in the management of the petty cash, individuals’ funds, program schedule and the Medication Administration Program systems.
    5. Assume Program Coordinator responsibilities in the absence of the Program Coordinator.
    6. Assist the Program Coordinator to develop program structures and routines which promote a therapeutic milieu which fosters growth and development for all.
    7. Participate in the development and accomplishment of the goals and objectives of the program.
    8. Provide Service Coordination for an assigned group of Persons.
    9. Assist the Program Coordinator in the completion of all Assessments and Treatment Plans according to prescribed timelines.
    10. Act as a liaison with community agencies and families to maintain coordination in the treatment process.
    11. Document Persons’ progress to maintain a permanent record of activity and treatment.
    12. Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with Persons served and family members.
    13. Demonstrate respect for all Persons regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity.
    14. Plan structured and supported leisure/recreational activities for Persons on evenings, weekends, and holidays.
    15. Provide side-by-side support and coaching to help Persons socialize.
    16. Facilitate and monitor the implementation of activities that create opportunities for the development of valued roles and personal relationships in the community.
    17. Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff. 
    18. Collaborate with Persons Served and Integrated Team members in the implementation and monitoring of any self management and health and wellness plans and activities.
    19. Intervene during a crisis, following established program and Agency procedures.
    20. Assist Persons to perform daily routines as needed, including personal hygiene and grooming tasks, household activities, cleaning, and laundry. 
    21. Provide nutritional education and assistance with meal planning, grocery shopping, and food preparation.
    22. Develop and monitor the implementation of all procedures that ensure the health, safety, and well being of individuals served.
    23. Develop and monitor all procedures that ensure the physical safety and cleanliness of the program.
    24. Collaborate with applicable Care Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of Persons are fully met.
    25. Assist Persons to access, and/or provide, all needed transportation.
    26. Participate in supervision, team, and other assigned meetings.
    27. Review and complete all required documentation.
    28. Attend and participate in all trainings, meetings and supervisions as assigned.
    29. Serve as the Person in Charge in the GLE in the absence of the Program Coordinator.
    30. To provide on-call coverage to ACCS programs as assigned.
    31. Acquire and maintain all required certifications.
    32. Perform other duties as assigned.


    Bachelor’s degree in Social Services with residential and administrative experience preferred.  One year of supervisory experience preferred.  High School degree, GED or equivalent and valid driver’s license required.
