Description and Responsibilities

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.

 

The Program Manager operates within the philosophies of Harm Reduction, Motivational Interviewing and the Housing First Model to assist individuals in meeting their needs and foster empowerment for the persons served. They oversee the Community Supports navigation services, including hiring, supervising, mentoring, and evaluating navigators. They also provide oversight of caseload management and staff assignments of individuals served, attend insurance provider meetings and develop collaborative relationships with those providers as well as assess healthcare needs and assist in the coordination and referral for healthcare services.

 

Other Key Responsibilities:

  • Monitor the billing of each navigator and ensure productivity requirements are met.
  • Oversee completion of all Assessments and Support Plans according to prescribed timelines and specifications
  • Assist in the development and accomplishment of program goals and objectives.
  • Work as liaison /advocate with employment programs, employers, therapists, doctors, and social security staff to assist persons served as needed.

Qualifications

  • High School Diploma, GED, or equivalent required.
  • Valid Driver’s License, acceptable driving record, and reliable vehicle with proof of insurance required. 
  • Minimum of two years’ professional or lived experience with housing supports and/or behavioral health, required.  
  • Previous supervisory experience in a professional setting required.
